Upon logging in by entering your user name and answering some initial questions, you will be offered a consent agreement to proceed with the EBP Inventory.
Important Instructions: From the time you sign in, you have 45 minutes in which to complete this EBP Inventory. Please plan accordingly before you start. The EBP Inventory has 21 items. We estimate that it will take you from 20 to 30 minutes to complete.
Also, please note that you cannot go back (using the Back button on your browser) and re-enter selections on previous pages. If you attempt to do so, you will receive an error message at the top of the page. You will however be allowed to continue on to the next question.
